Services are provided for a wide variety of diagnoses, including orthopedic, cardiac, neurological, and general medical cases. Our clinicians work to make the transition from hospital to home easier for the patient by getting them back on their feet as quickly as possible. The initial visit is the assessment/evaluation of the condition of the patient to develop the plan of care. The plan of care and treatments are designed with one goal, to help the patient improve their quality of life and independence.
Occupational Therapy services are provided for conditions in which upper motor activity is affected and requires assistance with activities of daily living. The goal of therapy is to assist the patient in becoming as independent in their own care and household as possible. Our therapists work closely under the orders of the patient's physician.
Services include:
- Assess and evaluate fine motor activity and treatment to improve function and quality of life.
- Instruction and training on dressing, meal preparation, and other activities of daily living.
Physical Therapy services are provided for conditions in which the patient's strength, mobility, or coordination interferes with the activities of daily living. The goal of therapy is to assist the patient in becoming as independent in their own care as possible. Our therapists work closely under the orders of the patient's physician.
Treatments provided may include:
• Therapeutic exercises to improve strength, joint range of motion, balance, endurance, and general reconditioning
• Ultrasound treatments
• Heat packs and ice packs
• Therapeutic Treatment (Muscle Stimulation, TENS)
•Prescribing individualized home exercise programs
• Home safety and fall prevention evaluation
• Bed mobility and transfer training to allow patients to move about their environment as freely as possible
• Ordering necessary medical devices or equipment
•Massage to decrease pain, muscle spasm, and/or edema, and to improve flexibility
•Educating patients about their diagnosis and prognosis
• Instructing family members and caregivers techniques and skills to better provide for the patient while allowing them to remain as independent as possible.
All treatments are designed with one goal: to help the patient improve their quality of life and independence.
Skilled nursing services are provided by specially trained and registered nurses (RN) and licensed practical nurses (LPN) who begin all cases with an assessment to develop a plan of care. The services are prescribed by a doctor on which may include such things as dressing changes (wound care), teaching and administration of medications, intravenous therapy, catheter care, diabetic care, disease process education and managment.
Teaching caregiving skills and techniques to family members is also incorporated into the plan of care.
